Services/Fee Schedule
Behind every service we provide is a dedicated team of professionals, each bringing their unique expertise and enthusiasm to our business.
Due to the increase time involved in setting up new patient charts, all new patient visits are $10.00 higher for the initial appointment. After the initial visit, the patient is considered "established" and no further visits include the increased fee.

| Established Patient: Home visit outside 10 mile radius | $60.00 + $1.00 per mile (one way only charged) |
| Trigger point injections: Established Patient In clinic or home Lidocaine/lidocaine + Ketoralac | $60.00/$70.00 + Mileage rates apply if outside 10 mile radius |
| IV therapy for headache: Home visit may incur mileage fee if >10 mile radius from clinic | $100.00 for 1 bag of fluids +$25.00 for second bag at same appt |
| K-12 Sports Physical | $25.00 (mileage cost waived if 2+ appts scheduled at same location) |
| Weight management: New Patient/Established Patient | $100.00/$80.00 Appontment Fee. Prescription medications sent to your pharmacy to fill |
| Proceedure: Laceration repair, splinting or I&D of abscess | Visit fee +$25.00 for supplies/procedure |
| Urinalysis/strep test | $10.00 |
